Consent & Release Form

Hard & Uncut ("Producer," "we," "us")

By signing below, I confirm the following:

1. Age Confirmation I am 18 years of age or older. I have provided valid proof of age if requested.

2. Nature of Content I understand that Hard & Uncut is a sex-positive interview series, and that I have been asked questions of a sexual and/or adult nature, which may include topics such as sexual anatomy, sexual experience, and sexual health. I am voluntarily participating and was free to decline any question at any time during the interview.

3. Grant of Rights I grant Hard & Uncut a perpetual, irrevocable, worldwide, royalty-free right and license to record, photograph, edit, reproduce, distribute, publicly display, broadcast, and otherwise use my name, likeness, voice, image, and statements captured during this interview ("the Content"), in whole or in part, in any and all media now known or later developed — including but not limited to social media (Instagram, X/Twitter, YouTube), our website, television, print, and promotional materials — for as long as Hard & Uncut chooses, without geographic or time restriction.

4. Editorial Control I understand that Hard & Uncut has full editorial discretion over the Content, including the right to edit, caption, or combine it with other footage, and that I will not be entitled to review or approve the final edited version before it is published.

5. No Compensation I understand that I will not receive payment or other compensation for my participation or for the use of the Content, unless otherwise agreed to separately in writing.

6. Release of Claims I release Hard & Uncut, its owners, employees, and agents from any and all claims, demands, or causes of action arising from the use of the Content, including but not limited to claims of defamation, invasion of privacy, or infringement of any right of publicity.

7. Governing Law This agreement is governed by the laws of the State of Georgia, regardless of the state in which filming actually took place, without regard to conflict-of-law principles.

8. Entire Agreement This form represents the entire agreement between myself and Hard & Uncut regarding my appearance and participation, and supersedes any prior verbal agreement on the same subject.
